Pharmacy will push the Physician Medication Reviews via Writi according to a pre-determined schedule. The PMR must be prepared prior to prescriber authorization by at least one staff member, which involves comparing the PMR orders to

A) The Resident’s Paper Chart, ensuring all orders and changes authorized since the last PMR are reflected on the current PMR

B) The Resident’s [e]MAR/[e]TAR, ensuring all orders correspond to the current PMR

This should include ensuring

  • All orders renewed on the last review are present on the current review and [e]MAR/TAR
  • All orders discontinued on the last review are not present on the current review and [e]MAR/TAR
  • All orders held on the last review are held on the current review and [e]MAR/TAR
  • All orders written after the last review up to the day that the prescriber signs the new review are reflected on the current review and [e]MAR/TAR
  • All resident information is current and complete

The Physician Medication Review may be manually updated as a result of the reconciliation process. Any new orders or changes after preparation *must* be updated on the PMR until prescriber authorization Once PMR is received at the Home via Writi, Nurses can start their Nurse Prep/Review, after the review is completed the physician will need to either Continue, D/C or Hold to the existing orders. Lastly once Doctor Processing has been completed the nurse’s will have to perform their processing (Drug Record, Care Plan, First Check, Second Check, Etc.) for the PMR’s.
